The forms* provided by Utah Filmmakers™ are formatted as fillable PDF documents—with the exception of signature fields which can be signed digitally on a computer or tablet—and can be printed before signing.

The documents are available for purchase through Gumroad.com which allows users to set their own price, paying an amount that they feel is fair for what they are getting. With a few exceptions, most of the documents have a minimum price of $1+. Proceeds go to the Utah Filmmakers™ Association, a 501(c)(3) nonprofit organization.
